I understand that participation in Spruce Fit Moms Kids Programs (including fitness classes, games, movement activities, indoor/outdoor play, and special events) involves inherent risks including but not limited to falls, collisions, strains, and other possible injuries.
I voluntarily assume all risks associated with my child’s participation.
I release and hold harmless Spruce Fit Moms, its owners, instructors, employees, contractors, volunteers, affiliates, and facility providers from any and all claims, liability, demands, damages, or causes of action arising out of my child’s participation, including those arising from negligence.
I confirm my child is physically able to participate in recreational and fitness activities.
I authorize Spruce Fit Moms to obtain emergency medical treatment for my child if necessary, and I understand I am responsible for any related expenses.
I understand that unsafe or disruptive behavior may result in removal from the program.
